Athletic Paperwork

As a Florida High School Athletic Association member school, all Mariner High student-athletes must complete an annual physical and athletic paperwork as well as meeting academic requirements. Starting with the 2021 school year we will be using athleticclearance.fhsaahome.org to manage our athletic paperwork online. Your parent or guardian will need to create an account and submit your paperwork online through the Athletic Clearance website; there is a helpful video on the homepage.

All paperwork must be filled out completely, accurately, and must be notarized then uploaded to your account in order to be eligible to participate in any athletic practice or event.
